Self Monitoring

Keep a Record. (Personal Diabetes Care Card PDF)
Keeping track of your important test and exams will help
you maintain good health.

Know Your Blood Sugar Numbers.
Check your own blood glucose level using a glucometer and write it down.
This is called Self-Monitoring Blood Glucose (SMBG) and should be done as
often as recommended by your health care provider. Below are some tools
to help you record your blood glucose and learn how to check your blood sugar.
