TOOLKIT Our mission is to assist health care practitioners facilitate diabetes care as recommended by the American Diabetes Association’s Clinical Practice Recommendations. NYDC developed the “Tools for Better Diabetes Care” toolkit to assist the physician develop a system to more efficiently address the multiple components of care necessary to manage diabetes. Also included are self-management tools to help patients keep track of important information and facilitate compliance with the course of treatment and screenings recommended by their doctor. Statewide Adoption In 2005, the New York State Department of Health Diabetes Prevention and Control Program (DPCP) elected to adopt the NYDC’s Guidelines for Adult Diabetes Care. In addition, several other components of the “Tools for Better Diabetes Care” have been adopted and sent to more than 25,000 health care professionals statewide as part of a comprehensive “Diabetes Prevention and Management” toolkit. The NYDC partnered with the DPCP in the assembly of this toolkit and the creation of several new management and prevention tools.
