TOOLKIT

The New York Diabetes Coalition’s (NYDC) Board of Directors are representatives from professional medicals societies, such as the American College of Physicians (New York chapter), Medical Society of the State of New York and the New York Academy of Family Physicians, health plan medical directors, the American Diabetes Association, quality improvement organizations and the state and county departments of health, and others.

Our mission is to assist health care practitioners facilitate diabetes care as recommended by the American Diabetes Association’s Clinical Practice Recommendations. NYDC developed the “Tools for Better Diabetes Care” toolkit to assist the physician develop a system to more efficiently address the multiple components of care necessary to manage diabetes. Also included are self-management tools to help patients keep track of important information and facilitate compliance with the course of treatment and screenings recommended by their doctor.

A primary component of the toolkit is the “Guidelines for Adult Diabetes Care.” By encapsulating clinical priorities in this one page tool, health care practitioners can more readily refer to proper care recommendations. The guideline has been endorsed by all members of the Coalition and adopted by many health care organizations.

Statewide Adoption

In 2005, the New York State Department of Health Diabetes Prevention and Control Program (DPCP) elected to adopt the NYDC’s Guidelines for Adult Diabetes Care. In addition, several other components of the “Tools for Better Diabetes Care” have been adopted and sent to more than 25,000 health care professionals statewide as part of a comprehensive “Diabetes Prevention and Management” toolkit. The NYDC partnered with the DPCP in the assembly of this toolkit and the creation of several new management and prevention tools.
