The guideline was developed to send a clear, consistent message about the minimum treatment protocols for managing the care for persons with diabetes. The guidelines are updated as necessary to maintain accordance with ADA recommendations.

Diabetes Mellitus Flow Sheet
A one page chart tracking tool which allows practitioners to record test values and treatment that follow the minimum practice recommendations. The flow sheet makes it easier for the practitioner to see a patient’s improving or declining trends, thereby making it easier to assess if a course of treatment requires modification.

Eye Examination Report for Diabetes
A two page form intended to increase and improve communication from the eye care specialist to the primary care physician regarding the results of a patient’s dilated eye exam. The primary care physician can give the form to the patient to bring to their next eye exam, be faxed to the eye doctor, or the form can initiate from the eye care provider. The form is approved by the Centers for Medicare and
Medicaid Services and may be used in lieu of a consultant’s letter to the referring physician. The form instructions are provided in both English and Spanish.

Personal Diabetes Care Card
This card helps patients keep a record of their diabetes care. It has spaces which allow patients to record test values and exams done at routine or quarterly visits, biannual, and annual exams and tests. The card provides space for important telephone numbers, such as primary care physician, foot specialist, and eye care specialist, and reminds patients that controlling diabetes is a joint effort between patient and doctor.
