• Western New York Diabetes Collaborative - The three health plans in Western New York, Community Blue, Independent Health and Univera, have been collaborating on improving diabetes care since 2001. Areas in which these plans have and continue to collaborate on are the adoption of the NYDC Guidelines for Adult Diabetes Care and promotion to network physicians, diabetes Continuing Medical Education programs and coordination of care issues related to the reporting of dilated eye examination reports to the patient’s primary care physician.
