New York Focus on Diabetes:

New York State Programs and Initiatives
The New York State Department of Health Diabetes Prevention and Control Program
(DPCP) collaborates with a growing number of internal and external partners as a recipient of the Center for Disease Control and Prevention's (CDC) Basic Implementation grant funding level. In order to accomplish the goals of the national objectives established by the CDC, the DPCP has established a number of initiatives:

  • New York Diabetes Coalition (NYDC)
    Diabetes Registry Project
    and Coalition Activities

  • Community Coalitions – The Community Coalitions for Diabetes Prevention form a cohort of 15 regionally based partnerships addressing both primary and secondary prevention of diabetes. The coalitions provide outreach, diabetes self-management education and community awareness events for at-risk populations, as well as professional education for providers who work with them. Primary prevention activities include lifestyle modification programs based on the findings of the Diabetes Prevention Program (DPP) clinical trial.

    On September 18, 2003, Health and Humans Services Secretary
    Tommy G. Thompson announced 12 grants totaling 13.7 million to
    promote community initiatives to promote health and prevent disease.
    New York State was selected as one of the 12 grants and four counties
    — Broome, Chautauqua, Jefferson and Rockland — were chosen to
    implement programs to reduce the burden of obesity, diabetes and asthma.

  • Preventing Type 2 Diabetes in Children -
    The DPCP has launched a new four and one-half year initiative addressing the prevention of type 2 diabetes in children. A competitive application process resulted in eight awards regionally disbursed across NYS. Contractors are focused on reducing risk factors for type 2 diabetes in children by increasing opportunities for physical activity and healthy food choices in the home, school and/or community.

  • Training Programs -
    The DPCP staff, in collaboration with the University at Albany School of Public Health, has designed and conducted several provider and/or public health professional training programs. They are available via web streaming on the NYSDOH website. Topics for the productions have included The Diabetes Epidemic: Preventing the Preventable; Diabetes in Children; Type 2 Diabetes Prevention in Children: Making the Case for Healthy Schools Approach; and Health Literacy.
