The New York Diabetes Coalition (NYDC, the Coalition)
is a collaboration of professional medical societies, health plans,
health-care focused agencies, and community organizations
dedicated to improving the health of people with diabetes.

Diabetes is a complicated disease requiring multiple components
of care for proper treatment. With excellent care and self-management,
many of the complications of diabetes including cardiovascular disease,
blindness, kidney failure and amputation can be prevented or delayed.
The NYDC has been working together since 1999 to develop educational
materials and diabetes management tools and to promote initiatives to
help people with diabetes and health care professionals manage
the complexities of diabetes care.
